Our Out Patients clinics Eden Valley Hospice currently provides an Outpatient Service running over 3 days, Tuesday, Wednesday and Thursdays. Referrals can be made by General Practitioners, District Nurses, Macmillan Team, Day Hospice, Eden Valley Hospice Ward, Oncology and other hospital teams. What do we provide?: ü An introduction to the Palliative Care Service ü Symptom control ü Alternative to admission ü Psychological support ü Family support ü Support for Primary Health Care Teams with “Sharing of Care” ü A limited domiciliary visiting service ü A link with Oncology Service
Referral Types: ü New patient assessment and introduction ü Day Hospice medical assessment prior to starting regular Day Hospice ü Follow up appointments for patients discharged from Eden Valley Hospice Ward ü Procedures; e.g. Infusions (Zoledronic acid) Transfusions Drainage e.g. Ascites New Patients are allocated an appointment of approx. 1 hour with follow up appointments ranging from 30-60 minutes. Every effort is made to immediately inform GP by telephone of any suggested medication changes and this is backed up with a dictated letter. When appropriate, some patients may be admitted directly to the Hospice ward from the Clinic. Patients are given contact details of the Clinic for an “open access” self referral in the future if needed. Onward referral: From the Clinic we are able to direct patients to other services when required e.g. Ward, Day Hospice, Macmillan Team, Occupational Therapy, Physiotherapy, Lymphoedema Nurse, Massage Therapy, Hospice at Home, Chaplaincy, Counsellor, Social Worker for welfare and financial advice and the Oncology Department. We hope to , subject to additional funding, ü Expand service to 4 days to give a range of morning or afternoon appointments; ü Expand the follow up service for Eden Valley Hospice discharged patients
